Healthcare Platform
A regional healthcare network needed a benchmark clinical data platform — one that could ingest, unify, and present patient data from 8 disparate EHR systems across 34 facilities, while meeting HIPAA requirements and clinician workflow demands simultaneously.
The challenge
Clinicians were logging into 3-4 separate systems per patient visit. Critical data — allergies, current medications, recent lab results — was siloed across systems with no guarantee of currency. In two documented incidents, medication conflicts went undetected because the prescribing system and the pharmacy system were not communicating. The risk was clinical, not just operational.
The solution
We designed a FHIR-native unified patient record as the single source of truth for all 34 facilities. The interface surfaces a clinician-prioritised view of patient data — critical alerts foremost, then active care plan, then historical context — in a visual hierarchy validated with 40+ clinicians across specialties. Systematic data-mining surfaces hidden patterns: unusual lab trend combinations, medication interaction risks, care gap flags.
Our process
-
01 Clinical Workflow Research
Observed 3 specialties across 5 facilities. Mapped 22 distinct clinician workflows and their data dependencies.
-
02 Data Unification Design
Designed FHIR R4 resource mappings for 8 EHR source systems. Built a conflict resolution model for contradictory records.
-
03 Priority Information Architecture
Created a clinical priority hierarchy validated by physicians, nurses, and pharmacists. Critical data surfaces first, always.
-
04 Pattern Detection UX
Designed the UX surface for ML-driven clinical flags — showing detected patterns with evidence, confidence scores, and suggested review actions.
-
05 HIPAA Compliance & Audit Trail
Built comprehensive audit logging with role-based data access controls reviewed by legal and compliance over 4 cycles.
Outcomes
- 1 systemUnified patient record across 34 facilities
- 34 minTime saved per clinician per shift
- 100%HIPAA compliance on launch audit
- 8 EHRsSource systems successfully integrated
